Parents at Christ Church C of E Primary School are asked to declare if their child has any health conditions or health issues on enrolment and then they are asked to complete a 'Contact and Medical Information Update' form annually.
Medicines can be administered in school but only if they have been prescribed. If parents wish their child to receive medication during the school day then parents must complete this form. On no account should children bring medicines into school or administer medicines to themselves.
If your child requires emergency medication e.g. an epipen or epilepsy medication, your child will require a health care plan. You must contact the school to discuss this and complete the relevant forms with the school nurse or SENCO.
Inhalers: Children suffering from asthma MUST keep inhalers in school. Please see that they are clearly marked and handed to the class teacher.
It is a parents responsibility to ensure that all medicines, inhalers and epi-pens are in school.
